Complete our health needs questionnaire!
If you complete our health needs questionnaire, you will be helping to improve the health and wellbeing of the visually impaired community in Sandwell. You could also win a £25 voucher!
Since our successful health and wellbeing event, SVI has been awarded more funding from the Primary Care Trust to analyse and produce a report on the health and wellbeing of the visually impaired community of Sandwell. Fifty Health Needs Questionnaires were completed at the event and three lucky SVI members won prizes, with the first prize being a £25 gift voucher to spend on healthy goodies at Boots.
SVI is about to launch the second round of questionnaires. We need at least another 50 to be completed in order that the information we get from the survey is accurate and fully represents the experience of the visually impaired community.
Please help us by completing a questionnaire – you can do this by:
Download the health needs questionnaire, complete and return via the SVI email address. Or if you wish you can print the form and return it using the SVI postal address and you can use Articles for the Blind to send the form.

Note: Closing date for completed questionnaires is 31May 2009 and the prize draw will be made at the SVI committee meeting in mid June – the lucky winner will be notified before the end of June 2009.
