A new vision strategy for Sandwell
Sandwell Council's Adult Services and Health is working with SVI (Sandwell Visually Impaired) an organisation run by visually impaired people, to develop a vision strategy for Sandwell.
Get involved!
We want to get the views of people with a visual impairment and the wider community, to find out what YOU think the main aims of a local vision strategy should be.
We also want to strengthen engagement with people with a visual impairment, and agencies that support those with visual impairment, in Sandwell.
Our consultation activities will run from 30 August 2010 to 17 November 2010.
How we are consulting
SVI will be visiting existing groups and forums across the borough, in addition to organising a range of focus groups and carrying out a questionnaire to gather local views.
You can download and print off the main questionnaire below, in PDF or Word format. There is also a short postcard-style form which has been designed to capture the main priorities for a vision strategy.
You can return your completed form to the freepost address details on the form, or by email if you prefer. The Word versions of the forms do not include images or tables to make sure they are accessible for people using screen readers.
